School-year Class Registration Form
FIRST: find your class(es) on our classes page.
Second: fill out a student section for each enrollment that you need.
This form will work for one, two, or three enrollements.


To register for art classes, please submit this completed form (as complete as is needed for whatever your situation is.), and use the link following to make your first payment as deposit. (Or, you may mail a check, payable to Firstlight, to our office at: 1710 General George Patton Dr, Suite 108, Brentwood, TN 37027). We require one tuition installment payment of $100 (2011-2010 classes) plus the once-per-year registration fee of $25 per student to reserve a spot ($125 total). Each school-year class meets 35 times - August through May. Your registration is a full commitment to the school-year class. Please read our policies for full information and to understand our operating proceedures. Email or call to check availability. There are many classes, so you should be able to find an open spot easily if you register soon!

SUPPLIES
Check this if you want to reserve a full set of supplies. You can pay AFTER you finish the registration form by clicking on the supply link at the top of each page. Or you can purchase at the studio on our supply days before your first class.
I would like to reserve a full set of supplies for each enrollment (now with hard case and bag to hold everything!).
Foundations supplies with bag and case: $178 plus tax
Toons supply kit: $138 plus tax
supplies should take you through the entire school year. 3 rolls of paper towels are not included, but required during the year. Special purchases such as stretched canvas are also not included, but available in our store at any time.
Commitment
(If you must cancel an enrollment, you may do so only by providing a 30-day advance written notice. No cancelations can be accepted after February 20, 2012 for school-year classes.
I understand that space is limited, and that by registering to reserve a 9-month place in a Firstlight class, I'm commiting to the full 35 classes from August through the end of May, (or remainder) for the above enrollment(s).
